I always appreciate the consults when a patient with a documented thrombocytopenia over the past 3 years is admitted with stable thrombocytopenia and they ask whether there are schistocytes.

It's apples and oranges, sorry Mike. Every country has a different testing and reporting regime. Pathology practices in Australia simply do not test mild or asymptomatic cases. We probably won't have the true picture until they do proper antibody tests for "clearances"

Is this just testing capacity differences or does the Detroit area have a higher proportion of positive cases?
— Matt Grossmann (@MattGrossmann) March 22, 2020
Yes - discussion always helps. Then we can convey the gray area that the case involves (like "They called it muscle invasion, but it's barely there" or "They said 3+4 but it's borderline". I always appreciate communication b.c. sometimes more clinical info can help.

Q7: At what time in the process should testing be performed? #pathologists #pulmpath #capchat
— CAPathologists (@Pathologists) January 16, 2019
it is frustrating but when you talk to pathology and at the moment testing capacity is overwhelmed - have to focus for now on where is can do the most immediate good, and plan/build extra cpacity in the meantime.
