The WHO does not mandate #ihcpath in the diagnosis of small cell carcinoma. Perhaps they need to be educated too.

So your cases is not a perfect parallel to the Pelosi cases, which started with resected carcinoids and looked back to see if any were diagnosed as small cell on biopsy.

My purpose is not to change the way you practice, just to relate how I practice, which is without Ki67

If you have any oversight of a lab, be very aware of the issue of interfering substances and help your clinicians to be aware of this as well. Lots of patients taking mega doses of biotin will not tell their physician "because it's just a vitamin."
— Matthew Wasco (@Gleason4plus5) December 17, 2019
Why is this? Because the way the test is conducted uses biotin as a co-factor, and the high levels from the mega-doses that patients take interferes with this. Depending on the chemistry, it can be falsely high OR falsely low.

Isn't EBV related mucocutanous ulcer a lymphoma? I would not consider it here. This is a spindle cell lesion. FDC Spossible by morphology, but this is a young kid. FDCS is common in mid-age patients.
