So your cases is not a perfect parallel to the Pelosi cases, which started with resected carcinoids and looked back to see if any were diagnosed as small cell on biopsy.

This is very pleomorphic with a hint of true keratinization. MEC is translocation related and rarely to be this ugly. other ddx would be Metastasis from skin or hn mucosal site...when u get surgical resection please update. Interested to see what this turns out as
