These histopathologic features;corrugated parakeratinized epithelium, palisading basal cells, daughter cyst,epithelial islands,are compatible with OKC-agree. Inflammation can be added secondary-which is not uncommon.

it is unusual for ATC to retain diffuse TTF-1 positivity (usually negative or only focal). PAX8 on the other hand can be positive in % of ATC. The way thyroid carcinoma dedifferentiated (ATC) is to lost TG first, then TTF-1, then PAX8, then all cytokeratin (usually).

While not obvious to some, as I see it, the larger problem is that it has been obvious to many of those 63M, but they 1) just don't care, and/or 2) don't appreciate the danger, and/or 3) are willing to accept the pathology and danger for secondary ($$$) gain.
