I personally think we're going a bit crazy with the focus on "cribriform pattern 4 vs non-cribriform pattern 4". A better option would be to more clearly define pattern 4, and perhaps exclude some of the "poorly-formed glands" that open the door to overcalling it by many.

Probably just a SCC.

MEC should not have overt keratinization & ++nuclear pleomorphism. CCC also in the differential but too pleomorphic. Fusion-driven tumor in general is monotonous! H&E not good for myoepi Ca, also it rarely (but not impossible) have true keratinization.
With diffuse s100 but neg melanocytic markers, the ddx are myoepithelial neoplasm with clear cell change and epithelioid mpnst. Look for PA or NF component by additional sampling and do CK, myoepithelial markers sma calponin and ini.

Salivary gland in the ddx: looks like there may be an outer layer of myoepithelial cells->intraductal ca vs. masc/salivary duct carcinoma..s100/AR/myoepithelial markers
Bone dry satire - my favourite sort.

That's an error. It should be 85097. 85060 is when peripheral smears are part of the biopsy
